Recovery Transition Meals

With generous support from Publix Super Market Charities, the Council on Aging provides 10 days of home-delivered meals following a senior’s discharge from a hospital, nursing home, or rehabilitation center.  Healing and a return to self-sufficiency hinge upon the time immediately following discharge.  From a study conducted by the Center for Medicare and Medicaid Services, we know that reliable, robust nutrition not only supports recovery, it also decreases readmission rates. 

Last year’s $6,000 grant from Publix provided sufficient funds to provide 10 days of this nutritional assistance to 86 seniors upon release from hospitals and other care centers.  Since receiving this year’s grant, 57 seniors have received this assistance and we are on pace to feed 29 more by September.  Together, the impact has been enormous—allowing independent seniors to resume daily life and move forward stronger than ever before.
