The Kane Clinic will offer the following benefits to Treasure Coast seniors, their caregivers and health care personnel:
- Elders and their caregivers will have increased access to preventive care.
- Elders with chronic diseases will experience easier and more frequent access to health care monitoring and information for chronic disease management.
- Patients will have better coordination between their health and community services through shared electronic records.
- Health system personnel will serve their patients better, using patient information from the Council on Aging's social service data.
- The Clinic will offer a model and training opportunity for community physicians and other care team members, and community outreach to those professionals who are seeking formal and informal education in the care of the geriatric patient. Such training and education will include a focus upon linkage to other services that alleviate isolation, boredom, anxiety, inactivity and unhealthy weight due to poor nutrition.
