The Council on Aging provides seniors and their families with a flexible array of cost-effective services to help them remain healthy and comfortable at home. We begin the process via phone, and then arrange for an in-home visit to assess the client's needs. Based on our assessment, we develop a comprehensive care plan, coordinate and arrange for services, and routinely monitor service delivery.
Our Help at Home services include:
- Companion services for socialization, physician appointments, shopping and other needs
- Homemaking
- Chore services to assist with seasonal heavy-duty cleaning
- Assistance with bathing and personal care
- Respite care and support for caregivers
- Assistance with bill paying and insurance/medical claims processing
- Long-term care planning
-
Home delivered meals through Meals on Wheels
- Adult day care
